Hello everyone! I just thought I'd share a simple tool I have developed that helps me in the assessment process. It is called a Support Person Survey. It's very simple, just two questions. I ask the client to take it home and bring it back the next day. They are given three surveys and are asked to get three people in their lives who know them well to complete them. I glean alot of really interesting and valuable information from this process. These are the two questions I use on the survey, of course I give ample space for responses:
1. What do you think are some of my strengths, skills and abilities?
2. What do you think are some of my weaknesses, needs or challenges that I face?
Hope this is useful to someone!
We need more postings on this blog, I know you all have some fabulous ideas you could share! Let's get this back up and running!

12. What kind of job or work does your child seem interested in or has your child talked about at home?

13. What are your child’s responsibilities at home?

14. Has your child had any previous volunteer experience or work training (with family members, relatives, friends, etc.)? If yes, please list them.

15. What does your child like to do most when not working or in school? (i.e. hobbies)

16. Are you aware of any issues/needs that might interfere with your child’s getting and holding a job?

17. Please add any other information you feel would be helpful to consider in planning and setting goals for your child’s future
